2013 Forms

Please complete these forms for 2013 Registration.   Returning Families will complete the Family Agreement online during sign up, but will still need to print, complete and sign the MHSL Insurance Form and have on file prior to the swimmer entering the water. Forms may be dropped off in the Family folder box at Kathleen's (7250 Brighton Pl).
